The prussik loop holding the tip steering line had become loose.	Injury Pilot: Unhurt

Found no fault and took off again only for engine cut out at 20ft. Pilot landed on knees but ok. This time found that fuel primer bulb was contracting after 30 seconds causing engine to cut out. Prop struck hand.	Injury Pilot: Severed index and middle fingers.

Date 01.08.2008 (B4279)	Pilot Male Age: 50 Rating: FLPA	Location Roshe, Cornwall, UK Wind Strength: 12-19 km/h Conditions: Variable	Wing Type Powered PG Wing: Paramania Revolution Launch Type: Powered Engine: Parajet, Macro	Summary At 400ft pilot parked brakes to adjust trimmers. He then hit turbulence causing the motor to move forward allowing the prop to touch the right brake. This wound on and the prop shattered damaging the cage. The brake then released allowing a safe landing.	Injury Pilot: Unhurt

Though protected by webbing fail safe, pilot and passenger decided to land immediately and did so without incident.	Injury Pilot: Unhurt

During this time one of the brakes became dislodged and was sucked into the prop. Pilot was able to land without incident.	Injury Pilot: Unhurt

Date 01.03.2008 (B4239)	Pilot Male Age: 58 Rating: FLPA	Location UK Wind Strength: 0-11 km/h Conditions: Smooth/Steady	Wing Type Powered PG Wing: Paramania Revolution 26 Launch Type: Powered Engine: PAP, ROS 100	Summary Pilot heard noise and turned to see that prusik loop holding wing tip steering line had slipped allowing line to contact prop causing damage to the line. Pilot landed safely and removed both lines.	Injury Pilot: Unhurt

Pilot was able to land with difficulty.	Injury Pilot: Unhurt
